PROCEDURE FOR ONLINE REGISTRATION

The online registration has SIX (6) main STEPS. They are Verification, Updating Biodata, Printing of Admission Letter and Other Forms, Fee Payment, Course Registration and Hostel Reservation. The first three (3) steps are just for printing admission letter and registration forms. If you have already updated your biodata or printed your admission letter, you can just proceed to Step 4.

1. VERIFICATION

a. Visit www.conamkat.edu.ng

b. Click “Student Portal”

c. Click “New Registration, Begin Here”

d. Put your Application Number (e.g BM201543) and click “Verify”.

NOTE: If admitted, it will display that you are admitted and it will redirect you to the page for updating Biodata.

2. UPDATING BIODATA

a. Enter the password you want to be using for login in the portal (e.g 123456)

b. Enter all your personal details.

c. Upload your passport

d. Click “Create Account”. It will show successful and will also display your password.

3. PRINTING OF ADMISSION LETTER AND REGISTRATION FORMS

a. Click on “Student Login”

b. Enter your Application Number and the password you put during updating of biodata and click “Login”

c. At the top left, Click on “Admission Letter” to print the Admission Letter (it should be printed COLORED and on a CREAM CONQUEROR paper)

d. Click on “Registration Forms” to download and print Acceptance Form, Library Form, Guarantor Form, ID Card Form, Undertaking Letter and Hostel Undertaking Form (It is a single PDF document that contains all the forms, it is 8 pages)

NOTE: Only Admission letter should be coloured. Those Registration Forms can be printed black and white on an A4 paper.

4. FEE PAYMENT

a. Click on “Fee Payment”

b. Click on “School Fee”

c. Click “Pay Now”

d. If you want to pay at the Bank, under “Cash Deposit at the bank” click on “Please Click here”. But if you wish to pay using ATM (preferably Master Card), under “Online Payment”, click on “Please Click Here”. And if you want to pay through USSD, copy the RRR number and put the transaction code on your phone.

e. After successfully payment, to print your Payment Receipt, Click on “Fee Payment”, then click “Payment History”, and then Click on the number under RRR (green in colour).

NOTE: After payment, the portal will issue you with a new registration number (e.g MFBM21001). The registration will now be the one to be used for all subsequent logins not the application number (e.g BM200001) but your password remains.

5. COURSE REGISTRATION

a. Click on “Course Registration”

b. Click on “Register”

c. Select all the Courses by clicking on “Check” (it is at the Top Right)

d. Click on “Preview Selected Courses”

e. Click on “Register Selected Courses”

f. Click on “Print Course Form” to print the course form.

6. HOSTEL RESERVATION

a. Click on “Hostel Reservation”

b. Click on “Make Payment”

c. Select “Hostel Accommodation”

d. Click “Pay Accommodation Fee”and make your payment.

e. To get room allocation, click “Hostel Allocation”, then against the Hostel’s name, click “View”, then a list of available rooms and bed-space will be displayed, so click “Allocate” against the one you choose.

f. To print Hostel Payment Receipt, click “Fee Payment”, then Click “Payment History”, click on the number (green in colour) against the hostel fee (3,600) under the RRR and print it.

g. To print the Hostel Allocation Form, Click on “Hostel Reservation”, then Click “My Hostel” and then Click “Print Hostel Allocation”, then Print it.

NOTE: Hostel accommodation is not available to Post Basic Nursing Students, but the College Satellite Campus can assist them to find accommodation in town.

Lastly, after finishing, ensure you fill and sign those REGISTRATION FORMS before going to the College Satellite Campus/for documentation. Also, your Parent/Guardian/Next of Kin should also fill and sign where provided. Make three (3) copies of each.

DOCUMENTATION AT THE COLLEGE SATELLITE CAMPUS

Take the following to the Admin Officer or Progamme Coordinator of your programme in your College Satellite Campus (original and 3 copies each) for documentation:

1. Admission Letter

2. Course Registration Form

3. Printed Payment Receipt

4. Completed Acceptance Form

5. Completed ID Card Form

6. Completed Undertaking Letter

7. Completed Guarantor’s Form

8. Completed Library Form

9. Completed Hostel Undertaking Form

10. Six (6) Passport Photographs

11. Acknowledgement Card

12. SSCE Certificate or Statement of Result

13. Secondary School Testimonial

14. Primary Certificate

15. Primary School Testimonial

16. Indigene Letter

17. Letter of Identification of Residence (If Community Midwifery)

18. Hostel Fee Payment Receipt (for those that will stay in hostel)

19. Hostel Allocation Form (for those that will stay in hostel)

20. Result Scratch Card(s)

21. One (1) A4 RIM Paper

NOTE:

1. If it is Post Basic Nursing student, in addition to the above, she must come along with 3 copies each of Licence, Notification of Registration, Certificate of Registration and Release Letter.

2. All students MUST collect a Medical Examination Form from the Heath Officer of the College Satellite Campus and take it to the designated hospital for tests. Students will be responsible for the payment of all the tests.
